Adoption form

Welcome to the Animals Lebanon adoption program, the place where all of our rescued cats and dogs find new loving homes. We request the following information to assist you in finding the perfect animal companion. Once you submit this form we will contact you to arrange a visit to our center to see all of the cats and dogs in our care. We then ask to visit your home to see where the animal lives and offer any advice to ensure the adoption is successful. Please note that

• Adopters are asked to make a $40 donation which is used to help cover veterinary expenses of the rescued animal.

• All cats and dogs in our care are safely neutered. This can decrease possible health problem later in the animals life as well as reducing the number of homeless companion animals.

• Cats and dogs can live to be 15 years or older and require annual vaccines and regular check-ups. You must be prepared for the ongoing responsibility of day to day care as well as the expenses for food and visits to the vet.

• Only the prospective adopter should fill out the adoption form, and you must be at least 16 years of age to adopt.
